- For intravenous treatments the price varies according to
what all is involved. Please call for pricing. - We will fill out insurance forms for clients to be reimbursed,
but we can not accept insurance as payment. - Medicare and Medicaid patients are welcome,
but Doctor is not enrolled in these programs. - Therefore no application for benefits
from these programs is legally permitted. - Please bring bottles of all prescribed
medications or drugs presently taken. - Include health foods, vitamins, herbs and other supplements.
- Please bring (if available) pertinent medical records such as:
most recent blood test reports, radiology reports,
consultation reports, hospital discharge summaries,
surgical reports, and pathology reports. - Doctor will copy the part he most needs
and return these records to the patient. - Patients may bring one or two caregivers along
with any appointment to help with the consultation.
